Project Details

Students will be organized into small teams of 6-7 individuals comprising of both university and secondary school students. Each team will be in-charge of caring for 2-3 elderly/households over a period of a year, before they handover their duty to the next team. We see the students playing some of the roles of the loved ones in

 

(1) Assessing the needs of the elderly

(2) Meeting these needs

(3) Coordinating the care of the elderly.

 

Each team will be making home visits to 2-3 households once every fortnight for at least a year, providing medical care and psychological support.

Medical Care

Regular blood pressure monitoring and blood glucose monitoring

Ensuring medication compliance

Fall risk assessment

Basic physiotherapy

Basic wound care

Health education (chronic diseases)

Empowering these individuals and their family in their own medical care

Psychological Support

Being befrienders

Coordinating the different financial services available to the financially needy
