Home Visits

Teams

Each team will consist of 2 to 3 Team Leaders, and 4 to 5 secondary schools students.

 

Team leaders would be the overall in-charge of the team. They will lead their teams during the home visits and be the point of contact for the team. They will also be responsible for the older adult’s coordination of care and wellbeing.

 

Assistant team leaders will be responsible for facilitating learning within the team and for the safety of the team. They will be responsible for taking care of the secondary school students and guiding them during the visits. They will also manage administrative duties within the team (e.g. Reflective log books for the students).

 

The participants or the secondary school students will participate in taking care of the elderly guided by the team leaders. They will assist with the duties of the team.

Frequency of visits

Each team will care for 2 – 3 households on a fortnightly basis. The team may be further divided into two groups (consisting of 1 leader and 2 participants) depending on the preference of the patients whom they are visiting.

Home Visit Protocol

First Visit

Team Leaders to facilitate the meeting and building of rapport and perform the Needs Assessment: Collection of biodata and relevant information (Questionnaire), Medical screening, functional assessment and psychosocial assessment. 

 

Second Visit

The entire team will be involved in meeting and building rapport. Needs assessment will be continued if it is not yet completed, and the team will then follow up on management plans.

 

Subsequent Visits

The groups (split into two) will perform tasks according to the team’s management plans.

For the Secondary School Students

The Tri-Generational HomeCare team has also designed a curriculum to aid in the learning process of the secondary school student volunteers. For each visit, there is to be time assigned at the end of the visit to review the learning objectives for the secondary school students, facilitated by the team leaders.
