Khoo Teck Puat Hospital (KTPH) Ageing-In-Place (AIP) Program

Spearheaded by Dr Wong Sweet Fun, AIP identifies patients who are ‘frequent flyers’, with a history of 3 hospital admissions in the last 6 months and a Community Nurse will be tasked to assess the reasons behind these admissions. It is our privilege to have Dr Wong Sweet Fun as our project mentor, to guide us and provide us with an insight into an area where students may not be exposed to. Besides medical issues, many of our patients have multiple social issues which propagated their medical problems, and finally leading to their hospitalisation. Together with the Medical Social Workers in AIP, NWCDC will be providing local assistance to patients who are unable to benefit from national social schemes.

National University of Singapore (NUS)

Tri-Generational HomeCare is made up of a diverse group of students from Medicine, Nursing, Pharmacy and Social Work, Physiotherapy, Occupational Therapy to offer different perspectives to the care of our patients. The intentional collaboration between the different faculties was inspired by the seamless inter-professional management of patients in the different hospitals.
